Dear Warsaw School Parent/Guardian,
In partnership with Super Shot, we will be providing in-school Flu vaccine this fall for Warsaw students on Friday, November 5, 2021.
When students are immunized against the flu, it keeps them healthy, prevents absenteeism, and keeps our entire school community healthy.
Most health insurance plans cover the cost of vaccines. Super Shot is required to bill insurance and will run eligibility per the completed consent. If the student has no insurance, there will be no out-of-pocket expenses. Under the Affordable Care Act, vaccines are covered without any cost-sharing/co-pay requirements (when provided by an in-network provider). Super Shot is in-network with all Hoosier Healthwise (Medicaid) providers and commercial insurance plans.
Please review the attached consent form to make sure your child meets the standards to receive the flu vaccine. If you answer “yes” to any of the screening questions, your child will not be eligible to receive the flu vaccine at school. We encourage you to take your child to his/her doctor or a Super Shot clinic for their flu shot. If your child meets the standards provided in the attached form, please complete the form and return it to school.
Please return the consent form to the school nurse’s office by Wednesday, October 20, 2021, for your child to receive a flu shot at school.
